Photography/Videography Release

I agree that the Coccidioidomycosis Study Group, has the right to use, replicate, and display any photographs (still, motion picture, or video) of me, including audio or readings of my voice, in educational audiotapes, videotapes, internet, television, public displays, or advertising related to the Coccidioidomycosis Study Group, and clients thereof.

Liability Release and Waiver

I agree to assume all risks associated with my attendance and participation in all on- and off-site activities that occur as part of the conference. By registering for this conference, I agree to indemnify and hold harmless Coccidioidomycosis Study Group and its governing bodies, officers and contractors from all loss, damage or liability arising out of, or related to, my attendance and participation.

Cancellation 

All registration cancellations and refund requests must be made in writing and submitted to the Coccidioidomycosis Study Group Office (support@coccistudygroup.org). A refund of the full conference fee, minus a $100 administrative fee, will be given for cancellations received before the posted deadline indicated in the registration.
